Athletic Registration and Forms
JCHS Student Athletes and Parents/Guardians: To participate in any athletic sport at James Caldwell High School, ALL consent and medical forms must be completed for each sport every season.
The deadlines for ALL paperwork to be turned in are as follows:
Fall Sports: July 15th
Winter Sports: October 15th
Spring Sports February 15th
Failure to turn in ALL necessary pre-participation physical exams, registration and consent forms and impact test will result in a MINIMUM of 7 days to process.
Registration forms are found in the Genesis Portal under Parent Forms

Remember to include an explanation for all items marked "yes" on the health history form; there is a space provided in the lower right corner of the form.
